Position Title: MFP Program Manager 

Hours: 8:00 am – 5:00pm M-F, some evenings and weekends, as needed  Position Type: Full-Time/Hourly 

Pay Rate: $23.00 – 25.00/hour 

Position Summary: The Program Manager is responsible for overseeing the Money Follows the  Person programs for Baltimore City, Baltimore County, and Harford County. The program  focuses on connecting with nursing home or rehabilitation center residents with disabilities and  offering education and support services for those that want to return to living independently in  their communities and providing support to the Maryland Access Point call centers in Baltimore  City and Baltimore County. The program manager is responsible for monitoring peer outreach,  peer support, and co-location efforts of the team members in the community. 

We are looking for an experienced professional who is willing and able to work in a structured  hybrid work model. The incumbent will be expected to work a portion of their week from home  and a portion of their week in the community visiting facilities, in Baltimore City, Baltimore County, and Harford County. The main office is located in Towson, Md.  

Key Responsibilities: 

• Provide outreach, education, and direct assistance to individuals with disabilities  regarding moving from a nursing home or rehabilitation facility to living  independently in the community. Carrying a caseload while overseeing staff. 

• Attending community events and workshops to promote awareness and education on independent living and accessibility solutions. 

• Maintain accurate records of client interactions, service referrals, facility visits. Including  data entry in multiple databases. 

• Oversee time and budget management for the department.  

• Ensure contract compliance and reporting requirements are met accurately and timely.  • Provide performance evaluations for team members. 

• Work with partner organizations as needed for the contract needs. 

• Attend IMAGE Center meetings and training as required. 

Qualifications: 

• Experience in community health work, social services, or disability advocacy. • Experience in program management and budget management. 

• Experience with staff management. 

• Experience working with the disability community.  

• Strong interpersonal and communication skills with the ability to build trust with diverse  populations. 

• Proficiency in record-keeping and client documentation in Excel and consumer record  databases. 

• Ability and willingness to travel throughout Maryland as needed.
